You can polish the lens without removing the headlight. Mask off the body and bumper. Or are you asking about removing the lens from the headlight? You have to heat it to soften the adhesive. Can't do that without removing the headlamp.
According to the service manual, bumper has to be removed. Looks like a couple screws are hiding under the bumper cover.

The lights work. Just the lens that's faded. Can I just pop off the lens without removing bumpers, etc?

You can not remove the front “lens” of the headlight. It is an integrated unit. Replacing the headlight requires removing the bumper cover, which is easy to do.
Many people restore the clouded headlight with a wet sanding and polishing procedure. Doing this will make them look like new, but you’ll have to repeat this process often. (Yearly?).

I replaced both my headlights a few years ago. It was very easy to do. OEM ones cost about $200 each.
 
I believe Meguiars makes a good protective coating that you can apply.

OR you can do the red***k method like I do before big trips.
Smear toothpaste all over the lens and let them sit overnight. Hose them down with your water hose the next morning, and you are good for a week or two. One of these day I will do it the right way like Chrisfix.

Only those who refurbish headlights for re-sales would do what you consider. There is youtube for that.
 
We got amazing results that lasted over 6 years already with the Spray Max 2K High Gloss Finish Clear Coat spray paint. We did three cars with just one can. Our cars are parked outside year round.

We do not know much about painting or body work, but this is what we did.

Put painter's tape around the headlight lens before wet sanding. Get the 800, 1000, 3000, 5000 and 10000 grit sand paper and wet sanded by hand. Do not use any attachment like a drill or buffer. Do not dry sand the lens. A combination of Harbor Freight and Amazon for the sand papers. We then use Meguiar's G17216 Ultimate Compound at the end before the clear coat. I think it took us three hours for all three cars. For one car, it might take 1 1/2 hour. The headlights came out almost like brand new.

It's theoretically possible since it started as a separate part, although the assembly would have to be removed from the van. The problem would be restoring the perfect factory seal. Realistically, removing the lens is not possible.

Anyway, the lens is not available anywhere, so removing it would be a dead end.

Your options are to replace the assembly or to refinish the lens.

I refinished mine using the Maguire's kit. It looked good, but didn't last long. So I replaced the assemblies. I regularly polish the lenses with the UV protectant left over from the kit.

It’s the clear lens that is cracked naturally of having it for a few years now.
