Update: replaced the entire accessory/serpentine belt tensioner assembly. The top pulley had a rubber wear mark at an angle from the new serpentine belt, and when the pulley was rotated by hand it appeared to be spinning at a slight angle. After pulling the pulley from the assembly and inspecting it, it's as if it was screwed to its backing just a tiny bit off-center. I installed an entirely-new idler pulley assembly from AutoZone, which is made by Dayco, part number 305369. The assembly I replaced was purchased off RockAuto around 1,000 miles ago.

I'll hold off saying the noise is fixed because I don't want to jinx anything.

If anyone else has a similar issue with this tensioner, when I removed the top pulley to inspect it, I could NOT re-attach the bearing. My theory is the bolt had become warped or the thread damaged due to use while being mis-aligned. Again, just my theory....
Get yourself a genuine Honda tensioner and belt.
