Serpentine Belt Keeps Slipping Off

Did this just suddenly happen with a belt that had been installed for a long time, or did you install a new belt?

if a new belt, are you sure you got the correct belt? Which engine do you have (VCM or non VCM)?

Im not sure if its VCM or not. I had to replace the entire belt tensioner assembly.

We also had to tap a new hole for The Tensioner Assembly.

One time the tensioner broke and my neighbor turned the Power Steering Pulley and Crankshaft to fit the belt that was almost fully on right before the tensioner nut broke.

That's why I'm thinking it could be one of those Pulleys

Then I had to take the belt off again because The VVT leak killed the Alternator.

It's like never ending with this van
Your mention of a "VVT leak killing the alternator" tells me you have the VCM engine which is on the EX-L and Touring models. The EX and LX models have the non-VCM engine.
